Elder Care Coordination

An Elder Care Coordinator is a licensed Geriatric Social Worker, who works for our Firm, bridging the gap between the Estate Plan and What Comes Next. Once a client has a comprehensive Estate Plan in place, our Elder Care Coordinator is able to help a family navigate the long-term care maze, and provide extra support and a helping hand to families during the long-term care process. Examples of what an Elder Care Coordinator does are:  hiring a home health aide, monitoring care, setting up the home for Elder care, applying for resources to pay for care, dealing with facility care meetings or discharge meetings, intervening with Medicare issues; intervening with family issues; and generally carrying out the Elder’s wishes in conjunction with the family.
